Insurance
We encourage all patients to call their primary insurance company for assistance in completion of the Insurance Questionnaire (available as a download under INFO SHEETS). Please bring this completed form and a current insurance card with you on the date of your first visit. We request that your co-payment or full payment as required by your insurance provider be made at the time of your appointment. At your request, we will file your insurance claim for you. Dr. Gentry belongs to many HMO and PPO networks. Please contact your insurance company to determine if our physicians are part of your specific network. Depending upon your insurance contract, you may be required to contact your insurance company and/or your primary care physician for approval of services that require referral/authorization. If our office can be of assistance in any of the above matters, please do not hesitate to contact us during normal business hours.
